Subretinal Hemorrhage Precedes Development of Angioid Streaks

A 15-YEAR-OLD girl noticed a paracentral scotoma in her left eye after performing a high dive. Complete eye examination showed visual acuity of 20/20 OU. The left eye showed a juxtapapillary subretinal hemorrhage and multiple yellow dots at the level of the retinal pigment epithelium throughout the posterior fundus (Figure 1 and Figure 2). The right eye showed similar yellow lesions.

Fluorescein angiography showed blocked fluorescence by the hemorrhage. Results of a physical examination were normal.

Examination 5 years later showed new, yellow, xanthomalike papules exhibiting a "plucked-chicken" appearance in the skin of the antecubital fossae and the neck. Angioid streaks now were apparent surrounding and radiating from each optic disc (Figure 3). The previously noted yellow dots were unchanged. A diagnosis of pseudoxanthoma elasticum was made.

COMMENT
