I SIGN the prescription. I do so with great reluctance. I believe that somehow I have failed and this is a treaty of surrender that I am signing.
The negotiation has been long and sometimes tedious, but this is the only thing that has made her "better." She has been having the episodes of shortness of breath, palpitations, and chest pain for many years. At first there were the emergency department visits and even admissions to the critical care unit. The results of stress tests, Holter monitoring, and other tests were all negative. Finally, the diagnoses of panic attacks and anxiety disorder were made, and the benzodiazepine treatment has lessened the frequency and intensity of these attacks. Nothing else seems to work—a trial of antidepressants gave her nightmares, and she has refused to see a psychiatrist—"I'm not crazy," she tells me. Still, I feel uneasy whenever I write that prescription.
