Nicht-suizidales selbstverletzendes Verhalten (NSSV) sowie suizidales Verhalten stellen insbesondere im Jugendalter weit verbreitete Phänomene dar. Der vorliegende Artikel gibt einen Überblick über die klinischen Kriterien und die diagnostische Symptomatik dieser Verhaltensweisen sowie möglicher Erklärungsansätze («gateway theory»; «third variable theory»; «theory of acquired capability for suicide») für die in der Literatur gut gesicherten Zusammenhänge zwischen NSSV und suizidalem Verhalten. Des Weiteren wird die Rolle von NSSV als sichtbarer Risikomarker für die Identifikation von Jugendlichen mit psychischen Problemen oder einem erhöhten Suizidrisiko diskutiert sowie beispielhaft bestehende populationsbasierte Selektionsverfahren (Columbia Suicide Screen; ProfScreen) erläutert. Insbesondere im Jugendalter kommt der Identifikation und Förderung des Hilfesuchverhaltens bei NSSV und suizidalem Verhalten eine besondere Rolle zu, um einer Chronifizierung dieser Verhaltensweisen vorzubeugen und negativen Effekten auf die psychische Gesundheit und das psychosoziale Funktionsniveau in der wichtigen Entwicklungsphase der Adoleszenz entgegenzuwirken.
