L’encéphalite auto immune (EAI) à anticorps anti-récepteur N-methyl-D-aspartate (R NMDA), la plus fréquente des EAI est rarement rapportée en Afrique Subsaharienne. Nous en rapportons le premier cas documenté du Togo chez une togolaise de 43 ans sans antécédent particulier admise pour des crises épileptiques généralisées inaugurales précédées de fièvre et de signes digestifs et suivies de troubles psychiatriques et de dyskinésies bucco faciales. Après une errance diagnostique, le diagnostic a été suggéré à J22 par le pattern d’extreme delta brush à l’EEG et confirmé par la présence des anticorps spécifiques dans le sang et dans le liquide cérébro spinal. L’évolution a été marquée par un décès dans un contexte de septicémie à J56.
