La patiente se présente de façon paucisymptomqique, avec une asthénie et une tachypnée, avec mise en évidence à la radiographie du thorax d'un volumineux épanchement péricardique et à l'échocardiographique des signes de tamponnade. Elle bénéficie d'un drainage en urgence, puis un bilan large à la recherche d'une étiologie, qui revient négatif. Elle évolue ensuite favorablement suite au drainage chirurgical et un traitement d'AINS et de colchicine. Nous illustrons dans cet article la démarche diagnostique à opter face à un épanchement péricardique, notamment l'explication quels examens complémentaires sont à effectuer, quels étiologies à rechercher, et quels traitements sont à mettre en place.</p> <p>ICMJE 6/6 ok - no COI</p>
