<p>Lipomas are common diseases of the skin, however, they can solely present in the chest. Intrathoracic lipoma is very rare in children, and most are asymptomatic for a long time. We present a previous healthy 5-year old child, who was presented in our emergency room due to a history of productive cough, rhinitis and daily high fever for a few days. The chest X-ray was misinterpreted as pneumonia with pleural effusion, and the child was treated with antibiotics. Due to persistent attenuated breath sounds and infiltration on the chest X-ray, a CT scan was performed, which revealed a large intrathoracic lipoma. The benigne tumor could easily been removed by the thoracic surgeon, and clinical and radiological follow up were uneventful.</p>
