Sei es, ob eine Tablette gemörsert oder eine Kapsel geöffnet werden kann oder aber wie ein Therapieplan letztlich in der Praxis umgesetzt werden soll. Die korrekte Medikamentengabe via Sonde ist für die Patientensicherheit und den Therapieerfolg von höchster Relevanz. Sämtliche involvierte Gesundheitsberufe sind aufgefordert interprofessionell zusammenzuarbeiten, um von der Verordnung (Ärzteschaft) über die Validierung (Apotheke) hin zur Durchführung (Pflege) eine sichere und effiziente Betreuung zu garantieren. In der Praxis bedeutet dies, dass für die Erstverordnung mittels einer strukturierten Medikationsanalyse die Therapie maximal vereinfacht wird und die Anwendung nur durch geschultes Personal umgesetzt wird.</p>
