      <abstract abstract-type="article" xml:lang="de">&lt;p&gt;Le diagnostic d'infection à C. difficile (CDI) reste un défi diagnostic, notamment en raison du manque d'un test diagnostique unique alliant sensibilité et spécificité élevées. Ainsi les dernières recommandations de l’ESCMID (European Society of Clinical Microbiology and Infectious Diesases) est l'utilisation d'un algorithme décisionnel en deux ou trois étapes. La 1ère étape de chaque algorithme comprend un test très sensible. En cas de résultat positif, le diagnostic de CDI doit être confirmé par un deuxième test de haute spécificité.
Un premier algorithme utilise la recherche de GDH par immunoassay ou des PCR comme 1ère étape suivi de la recherche de toxines A/B dans les selles par immunoassay pour la confirmation. Alors qu'un second algorithme comporte comme 1ère étape la recherche de toxine A/B et la recherche de GDH par immunoassay. En cas de résultat discordant lors de cette première étape, une PCR ou une culture toxigénique devrait être réalisé afin de confirmer ou infirmer le diagnostic de CDI. Le choix de l'algorithme est laissé à la discrétion des hôpitaux.&lt;/p&gt;</abstract>
