      <abstract abstract-type="article" xml:lang="de">&lt;p&gt;Belastungsabhängige Schmerzen im Unterschenkel sind eine verhältnismässig häufige Beschwerde. Eine Reihe verschiedener Differentialdiagnosen kommen in Frage, wobei bei Fehlen einer Grunderkrankung an ein mögliches CECS (chronic exertional compartment syndrome) gedacht werden sollte. Während das CECS früher als eher männliche Problematik gesehen wurde, so haben sich in der rezenten Literatur junge Sportlerinnen in Laufsportarten als weitere high-risk Gruppe etabliert. Für die Diagnose ist ein Belastungstest nötig, da die Befunde in Ruhe normal sein können. Gängige Referenzwerte sind ≥15 mmHG,  ≥30mmHG und  ≥20 mmHG vor, 1 Minute nach und 5 Minuten nach Anstrengung. Die Anstrengung kann dabei einfaches Laufen (eben, Treppen) sein. Wichtige Differentialdiagnosen des CECS sind die Periostitis, das tibiale Stresssyndrom ("shin splints") und seltener Nervenkompression, Bandscheibenläsionen mit Wurzelkompression oder die (funktionelle) Gefäßkompression. Weitere Grunderkrankungen können die Differentialdiagnostik erschweren (Diabetes &amp;amp; Neuropathie, Gefässerkrankung &amp;amp; vaskuläre Claudicatio, etc)&lt;/p&gt;</abstract>
