Exact indication of the publication from which you would like to reproduce material and detailed information about its intended use help to facilitate and expedite request processing. <p>Fallbeschreibung: Bei einem 69-jähriger Patient mit Parkinsonssyndrom, neu aufgetretener Blutungsneigung mit schwerer Anämie und Thrombozytopenie wird eine Raumforderung im vorderen Mediastinum diagnostiziert. Histologisch zeigt sich ein Thymom Typ AB mit assoziierte und in dieser Konstellation seltene Autoimmunphänomene: Pure Red Cell Aplasia, amegakariozytäre Thrombozytopenie und Good Syndrom bei Agammaglobulinämie. Trotz Resektion des Thymoms und immunsuppressive Therapie sind die Zytopenien therapierefraktär, der Patienten bleibt von Transfusionen abhängig und verstirbt.  Schlussfolgerung: Thymome sind die häufigste diagnostizierte Tumoren im vorderen Mediastinum und können operativ behandelt werden. Der Thymus hat eine zentrale Rolle in der Entwicklung von T-Lymphozyten mit adäquater Selbst-Toleranz, sodass bei Thymome Autoimmunphänomene aktiv gesucht werden müssen, bekannt ist die Myasthenia gravis. Als weitere Komplikationen können Zytopenien auftreten: am häufigsten Pure Red Cell Aplasia, Thrombozytopenie, Lymphopenie mit Agammaglobulinämie. Autoimmunmanifestationen der Haut sind auch beschrieben. Die Therapie der Autoimmunphänomene ist komplex und beinhaltet Immunsuppression (Cyclosporin, Rituximab), Substitution von Immunglobuline, Transfusionen und Infektionsprophylaxe. Die Prognose kann trotz Therapie ungünstig bleiben (gehäufte Infektionen, fehlendes Therapieansprechen).</p>
