      <abstract abstract-type="article" xml:lang="de">&lt;p&gt;Gastrointestinale Angiodysplasien (GIAD) sind nur für 2-8% aller Fälle von gastrointestinalen Blutungen verantwortlich (1). Mit ca. 40% sind sie jedoch eine häufige Ursache okkulter Blutungen beim älteren Menschen (2). Die Art der klinischen Manifestation reicht von akuten, hämodynamisch relevanten Blutungen bis hin zur schleichend auftretenden, chronischen Eisenmangelanämie. Die Behandlung von GIADs stellt bei den oft älteren, multimorbiden Patienten ein klinisches Problem dar. Der endoskopische oder chirurgische Therapieansatz allein ist wegen den oft multifokalen, schwer zugänglichen Blutungsquellen und der hohen Rezidivrate langfristig nicht immer wirksam (1, 3). Deshalb erfordert die erfolgreiche Behandlung von GIADs manchmal auch eine Systemtherapie (4-6). Anhand der vorliegenden Fallbeschreibung stellen wir einen multimodalen Therapieansatz vor, bei dem mit dem langwirksamen Somatostatin-Analog Octreotid auch eine solche Systemtherapie zum Einsatz kommt. Wir gehen in der Diskussion der Frage nach, wie Angiodysplasien entstehen, warum und wie Somatostatin-Analoga in dieser Situation wirken und welches die offenen Fragen bei der Behandlung dieser Krankheit sind.&lt;/p&gt;</abstract>
