      <abstract abstract-type="article" xml:lang="de">&lt;p&gt;Wir möchten mit unserem Fallbericht auf eine wichtige, wenn auch seltene Differentialdiagnose zum akuten Kopf- respektive Nackenschmerz aufmerksam machen. Die notfallmässige hausärztliche Zuweisung der 78-jährigen Patientin erfolgte aufgrund akut aufgetretener Nacken- und Kopfschmerzen, Schüttelfrost und deutlich erhöhtem C-reaktiven Protein (CRP) unter der Verdachtsdiagnose einer Meningitis. Klinisch zeigte sich eine febrile Patientin mit schmerzbedingter Nackensteifigkeit. Bei normwertiger Zellzahl im Liquor und fehlendem Nachweis einer Spondylodiszitis in der Magnetresonanztomographie (MRT) konnte letztlich erst computertomographisch der Nachweis von Kristallablagerungen im Ligamentum transversum atlantis erbracht und somit die Diagnose eines Crowned Dens Syndroms (CDS) als seltene Komplikation einer Calcium Pyrophosphat Deposition (CPPD) Erkrankung gestellt werden. Die Therapie erfolgte mittels nicht-steroidaler Antirheumatika (NSAR), worauf sich eine rasche und anhaltende Besserung einstellte.&lt;/p&gt;</abstract>
