      <abstract abstract-type="article" xml:lang="de">&lt;p&gt;Anhand eines illustrativen Fallbeispiels eines Patienten mit einer Raumforderung im oberen Mediastinum wird die Diagnose einer zentralen Atemwegsstenose Schritt für Schritt erarbeitet. Im Vordergrund steht bei der Präsentation des Falles der diagnostische Prozess mit entsprechenden «pitfalls» und differentialdiagnostischen Überlegungen. Eine zentrale Rolle spielt bei der Diagnostik die Spirometrie, auf welche detaillierter eingegangen wird. Sie ist in diesem Fall sehr hilfreich, da sich anhand der Form der Fluss-Volumen-Kurve eine intrathorakale Atemwegsstenose vermuten lässt. Im zweiten Teil wird auf spezielle Risiken eingegangen, welche bei der der Abklärung beachtet werden sollten, insbesondere das erhöhte Anästhesierisiko bei grossen mediastinalen Raumforderungen.&lt;/p&gt;</abstract>
