Exact indication of the publication from which you would like to reproduce material and detailed information about its intended use help to facilitate and expedite request processing. <p>Gastrointestinale Blutungen aus Varizen sind eine der häufigsten Komplikationen mit hoher Mortalität bei Patienten mit portaler Hypertonie. In über 95% bluten Varizen im gastroösophagealen Übergang oder Magen. In 1-5% blutet eine ektope Varize, welche ausserhalb des gastroösophagealen Überganges oder des Magens in verschiedenen Lokalisationen im gesamten Magendarmtrakt auftreten können. Wir beschreiben einen Patienten mit portaler Hypertonie, der sich mit einer kreislaufrelevanten Blutung aus einer ektopen Varize in einer choledochojejunalen Anastomose nach Lebertransplantation präsentierte. Nach ergebnisloser wiederholter oberer und unterer Endoskopie, CT-Angiographie und Kapselendoskopie, konnte die Blutungsquelle während einer Erythrozyten-Szintigraphie lokalisiert werden.  Da die Blutungsstelle über einen analen oder peroralen endoskopischen Zugang nicht sicher erreicht werden konnte wurde der Zugang chirurgisch nach einer medianen Laparatomie über eine Längsenterotomie 30cm distal der Hepatico-jejunostomie geschaffen. Die endoskopische Blutungsstillung erfolgte mit einem normalen Gastroskop durch eine herkömmlichen Gummibandligatur der blutenden Varize. Durch das kombinierte chirurgisch-endoskopische Vorgehen konnten das chirurgische Risiko und die Operationszeit im Vergleich zu einer Resektion der hepato-jejunalen Anastomose deutlich reduziert werden und ermöglichte eine technisch einfache und sichere endoskopische Intervention.</p>
