      <abstract abstract-type="article" xml:lang="de">&lt;p&gt;Apart from bile duct stones or tumors, the Lemmel Syndrome can be a rare cause for jaundice. It is a duodenal diverticulum next to the papilla that can eventually lead to pancreaticobiliary complications. We refer to a 70-year-old male who presents with pain in the lower sternal area. In the course he develops fever and jaundice associated to an increase in infection and cholestasis laboratory parameters. First we suspected a bile duct stone to be the origin of cholangitis, nevertheless the ultrasound examination could not detect one. An MRCP disclosed the secret of a large duodenal diverticulum. Due to an enclosed food bolus and inflammatory swelling, a cholangitis with bacteremia was caused.&lt;/p&gt;</abstract>
