Exact indication of the publication from which you would like to reproduce material and detailed information about its intended use help to facilitate and expedite request processing. <p>Wir berichten über einen 54-jährigen Patienten, der aufgrund einer unklaren systemischen Entzündung von uns untersucht wurde. Die führende Symptomatik waren ein persistierender trockener Husten, Sinusitis und B-Symptomatik. Der vorliegende Fall erwies sich bereits zu Beginn als eine Herausforderung, da die Bronchoskopie, die HNO-ärztlichen sowie bildgebenden Untersuchungen keine wegweisende Befunde zur Diagnosestellung zeigten. Nach erweiterten Laboranalysen und wiederholten histologischen Untersuchungen wurde eine Granulomatose mit Polyangiitis mit negativen ANCAs festgelegt. Anschliessend wurde das therapeutische Konzept entschieden. Hierbei sind die Schwere der Erkrankung und Organmanifestationen entscheidend. Liegt eine Organ- oder lebensbedrohende  Erkrankungssituation vor, dann muss eine intensive Induktionstherapie erfolgen. Hierfür sind zwei Therapieregime etabliert, die Behandlung mit Cyclophosphamid oder mit Rituximab. Eine Glucocorticoidtherapie und eine Therapie mit Cyclophosphamid wurde nach dem CYCLOPS-Schema angesetzt. Dies führte in den engmaschigen Verlaufskontrollen rasch zu einer Remission der Krankheit.</p>
