      <abstract abstract-type="article" xml:lang="de">&lt;p&gt;Dieses Fallbeispiel beschreibt exemplarisch eine diagnostische Herausforderung zur
Diagnose einer potentiell tödlichen Ursache eines FUOs.
Nach initialer Vorstellung mit rezidivierenden täglichen Fieberzacken, Husten und
einem Gewichtsverlust, ohne weitere wegweisenden Symptome, blieb initial eine breite
schrittweise Diagnostik der Differentialdiagnosen eines FUOs, inkl. infektiöse, nichtinfektiöse
entzündliche Krankheiten und Neoplasien, ohne Befund. Einzig zeigte
das PET-CT eine diskrete diffuse pulmonale Anreicherung. Die folgende invasive
Diagnostik mittels der Bronchoskopie und transbronchialer Biopsie erbrachte keine
Diagnose. Trotz diesen negativen Resultaten wurde bei progredienter pulmonaler
Klinik eine weitere invasive Diagnostik forciert, mittels Wedge-Resektion, mit nun
Diagnose eines intravaskulären Lymphoms, welches in etwa 50% der Fälle erst in der
Autopsie diagnostiziert wird. Dieser Fall zeigt, dass ein negatives PET-CT und
unauffällige Bronchoskopie ein Malignom nicht sicher ausschliesst. Dank der
klinischen Achtsamkeit und Forcierung weiterer repetitiver Diagnostik bei
persistierender bzw. progredienter Klinik konnte schlussendlich so durch eine
rechtzeitige Diagnose und Therapie die insgesamt schlechte Prognose deutlich
verbessert und ein potentielles Versterben verhindert werden.&lt;/p&gt;</abstract>
