        <p>Wenn bei Patienten multifokale neurologische Defizite auftreten und sich bildgebend als multilokuläre Infarkte darstellen denkt man in erster Linie an eine embolische Genese. Treten solche Ereignisse in kurzen Abständen auf oder zeigen einen progredienten Verlauf und lassen sich mit den Standardabklärungen wiederholt keine arteriellen oder kardialen Emboliequellen finden, so müssen andere Ursachen in die Differentialdiagnose einbezogen werden (siehe Tabelle 1). Sehr selten liegt dann ein im zentralen Nervensystem (ZNS) manifestiertes intravaskuläres B-Zell-Lymphom (IVBZL) vor.

Charakteristisch für das IVBZL ist das heterogene klinische Erscheinungsbild mit multifokalen neurologischen Defiziten, möglicher B-Symptomatik, das Fehlen eines Tumornachweises in der zerebralen Bildgebung sowie die unspezifische Labordiagnostik. Aufgrund der diagnostischen Schwierigkeiten wird die Diagnose oft erst spät im Krankheitsverlauf gestellt. Der hier präsentierte Fall eines histologisch gesicherten IVBZL dient zur Illustration der diagnostischen Herausforderung dieser prinzipiell behandelbaren Krankheit.</p>
