      <abstract abstract-type="article" xml:lang="de">
        <p>Bei der Ursachensuche einer intestinalen Perforation kommt eine Vielzahl an Differentialdiagnosen infrage, welche es im klinischen Alltag zu unterscheiden gilt. Nebst häufigen Ursachen ist es in gewissen Situationen - insbesondere bei der Behandlung von immunsupprimierten Patienten -  von Wichtigkeit, den Fokus auch auf seltene Ursachen zu richten. 
 Die vorliegende Fallvignette schildert den Verlauf einer 47-jährigen, infolge einer Nierentransplantation immunsupprimierten Patientin, welche nach Diagnose einer intestinalen Perforation zugewiesen wurde. Mittels erweiterter Diagnostik gelang es in interdisziplinärer Zusammenarbeit als Ursache eine intestinale Tuberkulose zu identifizieren und anschliessend erfolgreich zu behandeln. 
 Nebst epidemiologischen Aspekten zur Tuberkulose werden in der Falldiskussion schwerpunktmässig die klinische Präsentation und anschliessende Diagnostik diskutiert.   Zusammenfassend  lässt sich sagen, dass die Tuberkulose in unseren Breitengraden zwar selten ist, jedoch bei der Behandlung von immunsupprimierten Patienten durchaus in den differentialdiagnostischen Überlegungen berücksichtigt werden muss.</p>
