Das Wichtigste für die Praxis Die Leptospirose ist bei uns eine sehr seltene, aber wahrscheinlich unterdiagnostizierte Zoonose. Die Übertragung erfolgt durch Kontakt mit kontaminiertem Wasser über Hautverletzungen oder Schleimhäute. Die Leptospiren werden von infizierten Tieren über den Urin ausgeschieden. Nach einer Inkubationszeit von meist 7 – 10 Tagen variiert die klinische Manifestation von einer grippeähnlichen Erkrankung bis hin zum septischen Schock mit multiplen Organdysfunktionen. Konjunktivale Suffusionen sind ein typisches Zeichen. Die Diagnose wird mittels Serologieverlauf (MAT) gesichert. In der ersten Woche kann eine PCR aus Serum und Urin bestimmt werden. Blutkulturen sind wenig sensitiv. Bei klinischem Verdacht soll eine empirische Therapie erfolgen. Die Leptospirose beim Menschen gehört in der Schweiz nicht zu den meldepflichtigen Infektionskrankheiten. Infizierte Tiere werden gemeldet.
