        <p>Zuweisungen zur Ursachenabklärung einer Polyarthritis ist eine häufige Fragestellung in der rheumatologischen Praxis. Die Differentialdiagnostik ist breit darunter rheumatoide Arthritis, Psoriasisarthritis, reaktive Arthritiden, Kollagenosen oder auch seltener paraneoplastisch und stellt hiermit eine grosse Herausforderung dar. Trotz ausführlicher Anamnese, klinischer, laboranalytischer und konventionell-radiologischer Untersuchung kann die Ursache nicht immer gefunden werden. Das Gelenkpunktat, wie auch in diesem Morbus Whipple Fall, ist ein wichtiger Baustein in der Differentialdiagnostik. Der typische Verlauf des klassischen M. Whipple kann im Prodromalstadium mit einer Polyarthritis Monate bis Jahre vor Beginn der typischen gastrointestinalen Manifestationen und ZNS- Manifestationen sich zeigen. Hier kann es bis zu 50% der Fälle zu Fehldiagnosen kommen. Es ist daher sehr wichtig, auch in Anbetracht des möglichen tödlichen Verlaufes ohne antibiotischer Therapie, an den M. Whipple als Differentialdiagnose zu denken.</p>
