        <p>Eine 61-jährige Patientin wurde mit akuter Niereninsuffizienz und Hämokonzentration auf der Notfallstation vorstellig und entwickelte im kurzfristigen stationären Verlauf einen Schock mit Intensivpflichtigkeit. Die erweiterte Anamnese ergab, dass dies innert drei Jahren bereits die dritte Episode eines unerklärten Schockzustandes mit Akuter Niereninsuffizienz, Hämokonzentration und Hypalbuminämie war. Wir stellten daher nach Ausschluss anderer Ursachen die Diagnose eines Ideopathischen Kapillarleck-Syndroms. Dieses seltene Krankheitsbild beschreibt nicht anderweitig erklärbare Schübe von schwerer Kapillarendothelstörung mit massiver Plasmaverschiebung in den dritten Raum sowie im Verlauf plötzlicher Wiederherstellung der Kapillarfunktion und Rekrutierung des zuvor verschobenen Volumens. Die Akuttherapie ist bei mangelhafter Datenlage an die Therapie des septischen Schocks angelehnt wobei eine frühe Erkennung der Krankheitsphase hilft, Komplikationen wie z.B. Lungenödem oder Kompartementsyndrom vorzubeugen.</p>
