        <p>Das Wichtigste für die Praxis
- Die primäre Aktinomykose der Leber ist selten und imitiert klinisch und radiologisch eine Leberneoplasie oder einen Leberabszess.
- Bei chronisch-indolenten Erkrankungen, welche sich mit massenartigen Läsionen präsentieren, die auf kurze Antibiotikatherapie refraktär sind oder rezidivieren, muss differentialdiagnostisch an eine Aktinomykose gedacht werden.
- Die Diagnosestellung erfordert ein hohes Mass an klinischem Verdacht.
- Wesentlich für die Diagnose ist der histopathologische Nachweis von Drusen und filamentösen Bakterien, welche gezielt gesucht werden müssen.
- Die Therapie muss individualisiert werden und besteht meistens aus einer langen Antibiotikatherapie, die je nach Ausdehnung und Verlauf mit einem chirurgischen Vorgehen kombiniert wird.</p>
