      <abstract abstract-type="article" xml:lang="de">
        <p>Die VHL-Erkrankung ist eine seltene, autosomal-dominant erbliche Multisystemerkrankung mit hoher Penetranz, welche mit dem Auftreten verschiedener gut- und bösartiger Tumore einhergeht. Es handelt sich um ein Tumorprädispositionssyndrom, das durch pathogene Keimbahnvarianten im Tumorsuppressor-Gen  VHL  hervorgerufen wird. Die VHL-Läsionen finden sich vorwiegend im zentralen Nervensystem (ZNS), an der Netzhaut der Augen und in den viszeralen Organen. Standardisierte Untersuchungen helfen, Tumore bei VHL-Betroffenen in einem asymptomatischen Stadium zu erkennen und frühzeitig zu behandeln, um nicht betroffenes Organgewebe (z.B. Niere) zu erhalten, das Metastasierungsrisiko zu reduzieren und voranschreitende Defizite zu vermeiden (ZNS-Läsionen). Der VHL/HIF-Signalweg (HIF = Hypoxia inducible factor) spielt eine entscheidende Rolle in der Entstehung der VHL-assoziierten Tumore. Mit Belzutifan als Inhibitor von HIF-2α ergibt sich erstmals eine zielgerichtete Therapieoption für VHL-Betroffene, insbesondere bei Auftreten von Nierenzellkarzinomen und anderen VHL-assoziierten Läsionen.</p>
