Wir stellen einen 40-jährigen Mann vor, der akut erkrankte mit einem febrilen Ikterus. Es kam zum septischen Schock mit Mehrorganversagen (Kreislauf, Lunge, Nieren, Leber). Aufgrund der vertieften Anamnese mit Haltung von Ratten konnte schliesslich die Diagnose einer Leptospirose gestellt werden. Das schwere Krankheitsbild entsprach einem "Morbus Weil". Wir vermuteten eine kutane Infektion bei atopischer Dermatitis mit erregerhaltigem Rattenurin. Nach günstigem Verlauf konnte der Patient in die Rehabilitation entlassen werden.
