        <p>Ein Schlaganfall im jüngeren Lebensalter  ist selten und stellt das Behandlungssystem vor besondere diagnostische und therapeutische Herausforderungen. Die Ursachen sind heterogen und unterscheiden sich deutlich von denen älterer Patienten. Neben nicht-arteriosklerotischen Vasopathien, unterschiedlichen kardialen und mikroangiopathischen Ätiologien sollte auch an Drogenkonsum, Vaskulitiden oder Infektionen wie HIV oder Syphilis gedacht werden.  Gerade die Neurosyphilis stellt trotz des Erfolges der Penicillin-Therapie eine besondere diagnostische und therapeutische Herausforderung dar.  Die Entscheidung zur Durchführung einer Lumbalpunktion, die Interpretation von Liquorbefunden, klare diagnostische Richtlinien für Neurosyphilis, die Festlegung einer definitiven Therapie (einschließlich Alternativen zu Penicillin) und die Vorgehensweise bei der Nachsorge von Patienten mit Neurosyphilis sind mit Unsicherheiten behaftet.</p>
