<p>Face à un nouveau diagnostic de toux chronique, un traitement d’épreuve pour les trois diagnostics principaux (RGO, STOVAS, asthme), instaurés de manière successive ou en parallèle (puis retrait des médicaments un à un en cas d’amélioration) est indiqué. Si comme dans ce cas il n’y a pas d’amélioration, des examens spécialisés sont nécessaires. Au niveau gastroentérologique, il y a lieu de séparer les problématiques. Bien que la pangastrite sévère en présence de H. pylori doive être traitée, elle n’a pas de lien direct avec la toux. En cas de de toux persistante malgré un traitement d’IPP, c’est une impédance pH-métrie qui doit être proposée. Si en effet elle montre un reflux mixte acide et non-acide, il ne faut pas centrer l’approche seulement sur le traitement de l’acidité, mais aussi sur la prise en charge générale du RGO. Enfin, les syncopes à la toux contre-indiquent la conduite automobile, de sorte qu’avant que le patient ne soit à nouveau apte il faut que la cause sous-jacente soit bien traitée et/ou qu’une certaine durée depuis la dernière syncope se soit écoulée.</p>
