Le syndrome catastrophique des antiphospholipides est une complication rare mais sévère du syndrome des antiphospholipides. L'atteinte cardiaque, sous forme d'insuffisance cardiaque et/ou infarctus du myocarde, est présente dans la moitié des cas. Nous présentons l'histoire d'une patiente de 69 ans, présentant une insuffisance cardiaque aiguë à fraction d'éjection diminuée, associé à une atteinte rénale, pulmonaire, neurologique et cutanée. Nous retenons un syndrome catastrophique des antiphospholipides, traité par anticoagulation thérapeutique (héparine), corticothérapie systémique et séances de plasmaphérèse. L'évolution est rapidement favorable, avec récupération complète de la fonction cardiaque, sans examen cardiaque invasif effectué.
