Considerable additional work in these areas has been performed by the author and others since this paper was written two years ago. This additional work represents significant extensions to the material presented here, and will be reported in a subsequent paper. This work is licensed under a Creative Commons Attribution-Noncommercial-No Derivative Works 3.0 United States License.
